Loose, Unformed Stools, Occasionally Containing Yellow, Green Or Clear Substance. Advise?

Hi!
My 4 year old son was potty trained early and has been very independently using the bathroom for years now, so I rarely observe his bowel movements. Any time that I do happen to see, though, he has very loose, unformed stools, sometimes containing kind of a yellow/green/clear substance (bile? mucus?). I could not tell you the last time I saw a "normal-looking" BM from him. Also, because of the nature of his loose stools, about once every 2 weeks or so (on average) he has a small "accident" in his underwear. I believe he normally has about 1-2 BMs per day. This has been his "normal" for as long as I remember (the past couple of yrs anyway). Otherwise he seems very healthy. He is very tall and thin (like his parents), very energetic.
Any suggestions?

it looks your child suffers from recurrent diarrhoea.
normal stool colour is nearly brown.
as you are telling it is of yellow green colour which is not normal going towards some pathology .
most common cause is worm infestation . did u remeber when u gave antihelminthic drug to him , if not u should give course of albendazole .

second common cause is amoebiasis , you should go for stool examination if srool is positive for protozoa . u should give treatment accordingly.
