Painful Swallowing, Ear Pain That's Sharp. Taken Tylenol. Suggestion?

I have had painful swallowing, ear pain that's sharp and stabbing like for two weeks. No cold symptoms, no fever, smoke 7cigs a day, drink a lot of pop. No medication used except tylenol which helps a little bit.neck also feels funny like something is out of place close to the top of spine. Lost voice a few days ago for two days. Just feels like there a sharp object stuck in the left sidedle of my neck/throat. No insurance/money to see a dr.
