Pregnant, High Bp. Impression Of Presence Of Diastolic Notch In Right Uterine Artery And Increased PI And RI Of Both Uterine Arteries. Suggestion?

Hello mam, I am 30 weeks pregnancy with BP of 140/80, color Doppler at 30 weeks impression of presence of diastolic notch in right uterine artery and increased PI and RI of both uterine arteries suggestive of hampered materno-fetal circulation. RT uterine artery PI and RI are 1.70 and 0.79 respectively and LT uterine artery PI and RI are 1.16 and 0.62 respectively. I will be thankful to you for your opinion regarding outcome of pregnancy and risk associated with this finding and can pregnancy be full term and can this condition be treated to lead to full term.

The problem of high blood pressure in pregnancy especially if its the first is common. This will require close monitoring and medications can be suggested by your doctor as required.

The Uterine diastolic notch - diagnosed now, there is not proven treatment to change the findings
In such cases ideally a repeated doppler at regular intervals of the umbilical artery will be more beneficial to see the flow of blood into the fetus and measurements of the fetus to look for any alterations in the growth of the baby.

The pregnancy can be closely monitored and if the blood pressure can me controlled well and you respond to the treatment then it may be possible to carry the pregnancy to term and deliver

However if the symptoms do worsen and turn out unfavourable to the growing fetus then early deliver will be considered

Hello,
Thanks for posting your query.
Diastolic notch in uterine artery is initial sign of fetal compromise.
With these findings you can wait & get Doppler scan done at 4 weeks interval.
If findings remain same you can wait till 37 weeks & after fetal lung maturity can go for delivery.
Count daily fetal movements that should be more than 10 & keep in regular touch with your doctor.
