Should I Be Concerned About Child's Buttock Redness?

My 5 yr old son has a very red butt hole and it "feels like I have spikey poop". He poops one a day(around the same time) and it looks to be "normal". We have been treating it with A & D ointment when we remember but it just does not seem to go away completely. Should I be concerned and or what should I be looking for doing differently.

Deworming to be considered.
Ointment containing zinc oxide along with some steroid like beclamethasone to be applied three to four times locally. Hope it will help in reducing the inflammation.
