Before getting answers to all your queries, it is important for you to understand the natural history of the disease- Polycystic ovarian syndrome/disease (PCOD/PCOD).
In a normal monthly ovarian cycle, either ovary releases one ovum/ egg. In the development of this ovum a number of follicles mature and one develps to form a dominant folliclle. This dominant follicle re;eases the ovum per cycle. In case the ovum meets the sperm, conception takes place.
PCOD/
PCOS is a condition in which follicles develop but none of them matures into the dominant follicle to release the ovum/ egg. So, even if sperms are present there is no ovum to fertilize the ovum.
PCOD/PCOS is associated with
hormonal imbalance. LH (a hormone released by
anterior pituitary) is raised.
Estrogen (female hormone) levels are low and
androgens (male hormone) are high.
This hormonal imbalance may cause mentrual cycle irregularities.
The management includes:
1. Weight reduction
2. Metformin
3.
Clomiphene citrate- This drug stimulates maturation of the follicles in the ovary and hence release of ovum/ egg.
The preparation 'Ubiphene' contains clomiphene.
After starting with the treatment, follicular scan needs to be done. This scan tracks the maturation and hence release of the ovum.
Menstrual irregularities- less flow, irregular cycles can be treated with progesterones.
