Hello! I have stage 4 endometriosis and have had 3 laparoscopies to remove endometriosis adhesions. Today, they found a 8cm cyst on each ovary. My doctor is recommending surgery to remove them. Is surgery absolutely necessary for a cyst of this size? Will this surgery affect my fertility? Is it possible the ovary will need to be removed?
Hi Greetings I understand your concern. Stage 4 endometriosis and 2 Laproscopic surgery for removal of ovarian cyst and adhesions,would have compromised ovarian function partly. So I usually do a hormone AMH (Anti mullerian hormone) before the repeat survey in stage 4 endometriosis. Which help as to know the functions of ovary and the fertility potential. If the AMH is very low I would prefer to go ahead with IVF for fertilty because the third surgery might compromise ovarian function again. If AMH is satisfactory then either a cyst removal or cyst aspiration can be done Laproscopically and accordingly IVF can be advised as endometriosis cannot be cured. In stage 4 endometriosis it is better to go ahead with Assisted Reproductive Techniques rather than waiting after the third surgery. I hope you are convinced. Regards
