Hello. Thank you for writing to us

You seem to have a condition known as Melasma.
Melasma presents as brownish patches/ hyperpigmentation on sun exposed skin of face commonly on cheeks, dorsum of nose.
Melasma may affect other areas like chin, forehead and upper lip area as well.
Though you initially showed good response to the cream that you were using but you seem to have developed irritation to the topical cream and which possibly may have led to post-inflammatory hyperpigmentation Or a recurrence of melasma.
Sunscreen is an essential part of treatment any facial hyperpigmentation.
I suggest you to use a broad spectrum sunscreen, throughout the day. Reapply every 2-3 hours, specially when outdoors.
At night you may apply a pigment reducing cream containing one or more of the following ingredients like kojic acid, hydrioquinone, glycolic acid, azaleic acid etc.
Along with the topical treatment you may also opt for a few sessions of chemical peeling from a dermatologist. Glycolic acid peel is particularly good for pigmentation.

Hello,

I can understand your concern for sunburn marks over the affected area. I can get form your query that rather than the sunburn it is the application of the cream which is causing the main problem over the skin.
These creams contain Tretinoin in it and can cause irritation and later on pigmentation over the application area.
In such kind of cases it is quite important to avoid sun exposure as well as stoppage of the application of the Tretinoin containing creams.
Your doctor had given a good treatment and should follow for few weeks to get adequate results.
All the best.
