Suggest How To Change The Medication For A Minor Twitch.

A dose of 400 mg phentoin in two devided doses for 10 yrs after sx is given.
Then a minor twitch is observed . Mri is normal.
S. phenytoin leve is below 7.given emergency rx.
After 14 days it was bake to normal to 12 on same regimen.
Need to chnge on phytoin 300 mg er .
Advise on step by step option to change on it.

It seems that you are suffering from epilepsy seizures. You should know that phenytoin is a very good drug,but it may interact with many drugs and foods. That is why you have these fluctuations in plasma levels of phenytoin. If you are taking the dose of 400mg twice daily, I would recommend switching directly to one dose of 300mg ER in the morning and one dose of 300mg ER in the evening.
Anyway, considering these fluctuations of phenytoin in your blood and also its adverse effects when used for a long time, I would recommend considering other treatment options for epilepsy like Valproic acid or Levetiracetam, which have fewer adverse effects.
You should discuss with your doctor on the above treatment options.
