Suggest Medication For Nappy Rash

My daughter has had a nappy rash that has turned to blisters and is very red and raised it has not spread all over her body. hands mouth feet arms legs buttocks. My eldest daughter has now got the same thing. It started off in her nappy. They both dont have a fever but are very agitated. What could this be?
