General Surgeon 's  Response
Hi.
Thanks for your query.
Noted that you have pain in the right testis since the age of 10 and now your are 27 but the pain continues in spite of removal of the cyst on epididymis.
Now that there is a less erection and it goes down due to pain in the testis I would suggest you the following:
First of all to find the reason for the pain. This may be due to the pinched nerves supplying the testicular area and can be due to entrapment somewhere. If possible to find this and do neurectomy to abolish the pain so that the sexual function is improved and there is no pain at all. You may feel numb but he ultimate plan is to stop the pain.

General Surgeon Dr. Rishi's  Response
Hi,
Have you seen a Urologist.
I think that the pain is leading you to lose confidence in your sexual performance leading to a Psychogenic Erectile dysfunction.
You should get all possibilities of the reason of pain ruled out and then dont give much importance to the pain as it cannot be an impediment in your sex life.
Do consult a Urologist first.
Take care,
Dr Rishi, New Delhi, India.
