There are so many possibilities of decreasing cd4 and increase in viral
load
1. If the adherence to the therapy is not good...if you are missing dosage then take it regularly
2. If there is presence of any opportunistic infections .......if it tjere then first fo for its treatment
3Poor nutririon....as in your case your iron level has decreased....you might have poor nutrition.....include all iron reach food items in your diet like jaggery, dates, and all
vitamins rich food like fruits,green vegetables in your diet and pulses for protiens. Prefer xooked food then raw...as it will help in reducing g.i.t related opportunistic infection which is common in
hiv infected patients.
3. There may be immunological and virological failure due to resistance to current druf regimen.....in that case you may have to opt for second line
antiviral drug regimen....this is judge by decrease in cd4 count less than base line count which were at the time of initiation of therapy.......or below half of the peak cd4 count after starting of therapy.........or if
viral load is indicative of failure to current
regimen.
Anaemia is not the side effect of your drug regimen....iit might be due to iron and
folic acid deficiency........ so autrin given by your Dr. Is perfect.....if you are on clotrimoxazole prophylaxis then it is advisable to rule out
bone marrow suppression by doing CBC and blood indices.
