Suggest Treatment For Abdominal Distension

My significant other is age 49. He has never been able to develop much muscle tone since childhood. He is and has always been very thin. Even working in landscaping and having a very physical job in the past for 15 years never created muscles in arms or legs. However, his abdomen has become very distended over the last 8 years. He looks pregnant. He has regular bowel movements. Does not have a lot of gas but sometimes has Diverticulitis pain and diagnosis. He has been trying to lose the belly fat by cutting down on carbs and portions to no avail. He does have sleep issues, smokes minimally, and drinks alcohol. Should he see a specialist ?
