Hi,

According to your description, your son is having depressive mood and talks about suicide. This sounds like depressive phase of bipolar disorder. Zoloft is justified here to address the depressive and suicidal thoughts. But the risk is that the mood can move towards manic phase.

Risperidone is a very good atypical anti-psychotic. I believe that he is taking a 2mg tablet twice daily. Depakine and Lithium are both mood stabilizers, and their doses should be adjusted, and one of them should be chosen and raised to a higher dose. Lithium levels should be monitored regularly.

If I were your psychiatrist, I would recommend a combination of Lamotrigine and Quetiapine for the depressive phase of bipolar. If manic, then Zoloft should be stopped and your psychiatrist should see the possibility to stabilise the mood and increase or adjust anti-psychotic.

Take care. Hope I have answered your question. Let me know if I can assist you further.

Regards,
