In bipolar illness patient develop manic or depressive episode on different time. Treatment changes with time according to the nature of episode. Alcohol use can further complicate severity and frequency of episode.
Bipolar disorder need treatment under psychiatrist supervision with medication like mood stabilizers. Same time she need de addiction from alcohol. Most of the patient with bipolar disorder can be managed at home by medication and regular follow up with psychiatrist, so first seek regular treatment with psychiatrist for her, and if it is fail, then you can seek institutional care as per treating psychiatrist recommendation.
