ENT Specialist 's  Response
Hi,
my advice you is to consult ENT surgeon, Gastroenterologist at the earliest for ruling out Aero digestive tract malignancies with or without mediastinal compression and secondaries in lymphnodes.
The hiatus hernia with GERD problem itself may cause hoarseness, diff in swallowing and breathing. the bump mentioned by you in neck and bluish discolourations or warning signs.
Please take care.
