Suggest Treatment For Bowel Dysfunction In A Child

my 19 month old daughter has a bowel movement dysfunction and the Dr.s here don't know whats going on and wont tell me anything can you help me? she isn't constipated she goes to the bathroom but its like a bunch of little poops that clump into one big lump and when she pushes it out it hurts her so bad that it tares her little rectum open and bleeds, it also hurts her when i wipe her after wards. what should i do? i tried giving her fruits and juice such as grape, apple, and prune juices. i also tried the Kayro Syrup in warm water none of these are helping her. she also has bottle decay i give her fluoride every night before she goes to bed after i brush her teeth? i don't know what else to do?
