I have used Spiriva twice for conflicting diagnoses of chronic asthma or COPD. Oral Prednisone did not work for me and neither did Spiriva. I take Ventolin Prn and Symbicort BD and my peak flow is never better than 190 and is usually lower. I have very poor lung function tests, most recently only managing the score of a 90 yr old and I am 62. I am allergic to most antibiotics. My recent chest xray was clear tho I have hyperinflated lungs and a small granuloma which I was told was a result of being exposed to tuberculosis at some stage. I have had lung issues all my life and was diagnosed with asthma in my late 20 s. What treatment would you suggest for me please Doctor? I ll have to leave it cos I don t have money and didn t realise I had to pay for your help.
Hello dear, thanks for your question on HCM. COPD ( Chronic Obstructive Pulmonary Disease ) is progressive disease affecting lung functions. It will need lifestyle modifications and inhaled drugs (bronchodilators). Treatment of COPD is as follows. You need to do all the steps simultaneously to achieve better control. 1. Quit smoking if you are smoker. 2. Start triple drug in inhalation. LABA ( long acting beta 2 agonist ) and inhaled corticosteroids ( ICS ) with anticholinergic drug. 3.Enroll in pulmonary rehabilitation center where Chest physiotherapy and deep breathing exercise Are done. 4. Take adult respiratory vaccines like pneumococcal and influenza vaccines. They will prevent future lung infections. So better to consult pulmonologist and start all these simultaneously.
