I understand that you are going through a difficult time due to your depression and anxiety problems. Most of the symptoms that you have described are suggestive of a Mixed Anxiety Depessive disorder.
Don't worry, there are effective treatment options available to control your symptoms. There are medication called SSRIs which have good antidepressant and antianxiety properties. In addition psychological therapies like Cognitive Behavioural Therapy (CBT) can also be very helpful. So, please consult a psychiatrist for a detailed evaluation and further treatment.
