I had non specific white matter hyperintensities show on a MRI scan . I suffer from terrible headaches. They are getting worse as time goes on. I am nearly 45. I have dull headaches virtually all the time and then once or twice I week I get really bad headaches. The pain is intolerable. It got to a point where over the counter meds were no longer effective. My nose starts feeling blocked as well. However the scan said my sinuses were normal. My eyes feel pressured(eyesight is normal). I cannot sleep it off . I am nauseous and I feel faint, hot and sweaty. Lately I have become forgetful . My memories are mixed up and I have foggy recall - it feel like there is a curtain. I get angry and agitated a lot more now. I can t sometimes find a specific word for something and I take 2/3 sentences to describe something I could have described in one word. I use wrong words like the other day I wanted to say spoon and I said door. I am increasingly relying in lists. I forget I have done things - eg checked for the gas, switched light off etc. I am a lawyer by profession but I can feel I am less sharp- I can t remember case details , so I have to reread a lot more now. I am clinically depressed.

Your dull headache, along with episodic severe pain could occur due to migraine, tension headache or may be due to depression. So to make proper diagnosis you need to consult a psychiatrist. The symptoms like forgetfulness, anger and irritability need evaluation. You are 45 and have symptoms like forgetting words, forgetting day to day tasks can occur due to some cognitive dysfunction or may be due to dementia.
You should consult a psychiatrist for evaluation of these symptoms. MMSE or mini mental status examination is commonly used to find out cognitive deficits in individual. For depressive symptoms medicines like SSRIs or SNRIs are commonly used. Visit a psychiatrist for detailed prescription and evaluation.
