Hi,I am Dr. Subhankar Chakraborty (Gastroenterologist). I will be looking into your question and guiding you through the process. Please write your question below.
Yes, I am looking for an independent proctologist who might give me a second opinion regarding a really bad hemorrhoid/fissure problem. I just had a colonoscopy at Kaiser and I was told I indeed had both mentioned earlier (NO NEWS FLASH FOR I AM OUT OF COMMISSION TO SAY THE LEAST). Kaiser is dragging their asses to say the least while I suffer every day, so I would like a second opinion as to what an MD might find outside of Kaiser and WHAT CAN IF ANYTHING CAN BE DONE. I have reached a point where my daily life is being negatively effected. The bathroom is no fun!!! I need to see someone in Santa Clara/San Jose/ Sunnyvale/Cupertino etc for a second opinion..................ps I do have the Kaiser report......Harry R. Ruggiero Sunnyvale 408 730 2225
Anal fissures form as a result of high pressure within the anal canal (usually from constipation/straining) which leads to ischemia of the anal tissue and splitting of the anal mucosa during defection.
I would recommend: -All measures to prevent constipation; avoid straining and prolonged sitting on toilet. -Wash area gently with warm water -Stool softeners such as docusate daily -consume high-fiber diet and add daily fiber supplement such as psyllium -increase fluids -maintain healthy weight. -you doctor can prescribe lidocaine gel for topical analgesics -Sitz baths -Most acute fissures heal within 6 weeks with conservative therapy. -You need to be screened for infectious diseases or Crohn disease
