Thanks for posting your concern in HCM. You haven't mentioned the sugar level. With the present information, I would suggest you to have a complete blood count, CRP, urea, creatinine and electrolytes. Infusion of IV fluid can be judicially after assessment of any third space collection and volume overload. IV antibiotics may also be required depending upon the white cell count and CRP. In case of suspicion of an infection, the infective focus needs to be found out . A chest xray and urine RE may be required to depending upon the clinical findings. Later on, blood culture may be required if sepsis is suspected. My recommendation is to admit the patient and in patient treatment with close monitoring of glucose levels and vitals. Best wishes. Regards.

General & Family Physician Dr. Behar Greca's  Response
Hi, thank you for posting.

I read your query carefully and I understand your concerns.

I recommend you to follow these guidelines in order to keep Diabetes mellitus under control:
1. Avoid carbohydrates from your diet.
2. Avoid foods that contain a lot of cholesterol, such as fats and egg yolks.
3. Do not eat potatoes, because they contain carbohydrates.
4. Do not eat sweets and desserts.
5. Eat a lot of vegetables and fruits that don't contain a lot of sugar.

To treat this condition you can take Metformin, which prevents the absorption of carbohydrates. Along with that, Glibenclamide can also be taken.

You should also consult your endocrinologist.

Feel free to ask me for anything unclear.

Take care,
Dr. Behar.
