Noted the history about your friend who had to have a meh lining implanted to treat a large
hernia. His body is rejecting the lining, has a severe infection that has that has opened up the whole on the outside of his stomach that goes all the way through to his stomach. He is on antibiotic but the wound is not healing. Wants to know the further course of action.
I would advise him the following:
First of all, to get all the relevant investigations done to find out the reasons for non-healing:
- Tests of blood:
Hemoglobin, whether there is
anemia, WBC for level of an active infection, Blood sugar to rule out
diabetes, Serum proteins to see whether hypo-proteinemia. We need to correct all such deficiencies for the body to have healing power.

Blood culture ans sensitivity helps in some cases.
- Pus from the tract for gram staining, AFB staining for
tuberculosis, culture and sensitivity.
- Sinograon to see the depth of the
sinus or a fistula.
All these investigations will help to assess the case and to have the further course of actions.
Preliminary exploration of the tract and to remove all the granulation tissue and dead necrotic material is of primary importance to have better healing.
Repeated till cured, antibiotics as per the reports and such things all help to have a proper healing.
A gap of 9 months is needed between healed tracts and then only a mesh can be put-in to have better results.
