Suggest Treatment For Irregular Heart Rate And Low BP

My husband has congestive heart failure. He has has 20% EF for 5 1/2 years now. He has had few problems as he takes good care of himself and follows all doctors directions. He just got out of the hospital after being treated for fluid and shortness of breath. He lost 14 lbs of fluid in 3 days. His heart rate is not regular ( 46-109) and blood pressure is low (74/52- 99/62). Doctors say his heart is very poor. My husband feels pretty good except for being very, very tired. He is 69 years old. Is it possible to for him to be in this poor condition without many of the symptoms that we were told to expect?

Hi welcome to HCM
Yes patients of congestive heart failure can be asymptomatic and there could be sudden exacerbations leading to sudden cardiac arrest. So he should be monitored regularly. He should continue with all his current medications. Regular monitoring of ECG is mandate as his heart rate is irregular which may lead to heart blocks. Drugs like dopamine , dobutamine will be helpful. Also get coronary angiogram to know the risk of coronary artery disease by speaking to your cardiologist . Monitor his BP and heart rate daily . Report to your doctor immediately if he has fluctuations in BP , sudden shortness of breath , dizziness . Consult your cardiologist for expert management.
Thankyou.
