Suggest Treatment For Muscle Cramps,dryness In The Eyes And Abdominal Pain

Hi, below is my story. Can you please help me:



Age: 29, Sex: Male, Height: 5’10”, Weight: 178 lbs, Race: White (Middle Eastern)

Personal Past History: Healthy

Personal Family History:

•     Younger brother was diagnosed with hyperthyroidism (probably Grave’s) in 2011 age 27, Father diagnosed with Discoid Lupus in 1995, Father diagnosed with DM-Type II in 2010

Chronological order of symptoms since occurrence:

1-     August 2009: Painless left side middle ear infection. Persisted for two months. Treated with CiproDex (I used it for more than a month as opposed to five days that was prescribed)

2-     August 2009: Twitching in left eyelid for 3 months (resolved now except for occasional twitches)

3-     November 2009: Woke up with a funny neck position with pain at neck and right shoulder which has still not fully resolved. I also had a funny sensation in my throat (like a small ball is there)

4-     November 2009: Three consecutive panic attacks with heart rate 180+ bpm rushed to hospital diagnosed with panic attacks (no more panic attacks since then)

5-     February 2010: Body wide twitching. Diagnosed with depression by family doctor. Took Wellbutrin and Ativan for 3 months. Meds caused mental disturbances and severe abdominal pain. Discontinued meds. Body wide twitching has since improved 90% (occasional twitches)

6-     February 2010: Diagnosed with hyperthyroidism at Sunnybrook hospital. Treated with Propranolol. Hyperthyroidism resolved spontaneously within two months.

7-     March 2010: Occasional numbness and tingling at both legs.

8-     April 2010: upon discontinuation of Ativan and Wellbutrin severe burning/freezing pain at areas previously affected by numbness and tingling. Also burning pain at right shoulder and arm. Legs feel cold at times but they are not actually cold and sometimes are actually cold compared to other limbs

9-     May 2010: Noticed dry eyes (occasional) especially left eye which became completely dry at several occasions. Also noticed dry nostrils (especially right nostril).

10-     June 2010 – present: Burning/freezing/aching pain and muscle cramps in both legs, buttocks and right shoulder and dryness of eyes/nostrils/throat continue

Notes:

•     Pain is a constant 5/10 and at times becomes 8/10 aching burning pain

•     All above symptoms in addition to GI symptoms (indigestion, heartburn, diarrhea) fluctuate in intensity up-down-up.

•     All symptoms worsen and improve in episodes

•     Coffee, Cigarettes (even second hand smoke), tea, coke (drink), alcohol almost immediately and significantly worsen all symptoms

Tests so far:

•     Neurological examination by two neurologists: very brisk reflexes (symmetrical), 3 ankle clonus, normal otherwise

•     Brain MRI, NCS/EMG Normal

•     Blood work CBC, ESR, CRP, ANA, RF, SS-A, SS-B, HIV, VDRL negative

•     Chest X-ray: Normal
