Suggest Treatment For Nose Bleeding

My mother in law had sinus surgery 18 months ago. She said she never did breath as well after that. Today she told us she blew out a very large, pinky size, blood clot just before lunch. It has been bleeding for five hours. She s kept a tissue plug in, changing frequently. I had keep one for an hour to see if it was interpreting clotting to keep changing. She also has dementia, so it s hard to know the exact details before my husband picked her up at one. Should we take her to an er?
