Suggest Treatment For Pain Post A Lobectomy Lung Surgery

Person is experiencing pain, due to post-op right lung lobectomy, a month ago. Treated with acetamenophen ativan and tramadol. He s also experiencing nausea. Temp. is 101. Last night it was 99F. Bp is 124/88, pulse, 100. Last night he had a big dinner. Today in bed all day with fatigue and nausea. HIs oxygen is 93. His urine is very dark and pungent. His breath smells bad as well like vomitus. He is fearful of constipation due to opioid and takes stool softener, Has taken Mylanta at times for indigestion and constipation. He is also post op bladder cancer, with urostomy bag. Has a history of sepsis following the bladder surgery in December 2016: cystectomy, prostectomy with ileus from small intestine. His pathology shows no evidence of cancer in lymphs or lung. Cancer in bladder was gone. He was treated with chemotherapy last year, cisplatin and gemcitabine (sp?). My action is to check vitals, if temp increases and b/p decreases then will take him to the ER for ? of sepsis. Thanks.

The possible reason here is respiratory infection with urinary sepsis. Even though if vital parameters are normal, as per the symptomatic severity he needs a visit to ER without delay to get the precise evaluation and management guidelines.
