Suggest Treatment For Patient Talking To Themselves And Debilitating

Someone I know is talking and debating with themselves a lot! They are male, 46, a heavy smoker and previously had an angioplastiogram. They are withdrawn and it is difficult to communicate with them sometimes. I feel they don't listen. Can you help?
