I read your question carefully and I understand your concern.
I think in order to know what to expect you should understand first what
stroke means. What happened is that a blood vessel was blocked by clot. As a result there was no blood to the brain area it supplied. Now, if there is no blood supply that area (as any part of our body) can only survive for some hours, afterwards it dies, there is no rescuing it even if blood supply is restored later. Whatever improvement there is it will be thanks to remaining brain creating new connections to compensate, there is no drug to reverse that dead area.
So to answer your question:
1. There is no proven medication for
aphasia, only
speech therapy can help somewhat. Drugs like Strozma plus, remac OD try to enhance the remaining brain function but their benefit is not proven. As for
Ecosprin and Telact their aim is to prevent other strokes from happening in the future, not treat the current stroke, no drug can now.
2. The damage is already done now, so whether the clot is removed or not doesn't matter anymore. Actually the clot dissolves over the first few days, shouldn't be there anymore, but as I said damage is done.
3. There can't be made any predictions, most improvement happens over the first 6 months, if not till then you should face the possibility some damage will remain.
So, while I understand my answer might not be very heartening, you should try to be realistic and know what to expect. I am not saying there is not to be expected any improvement, judging by infarct size on
MRI there is hope, but improvement won't come through drugs but through physical and speech therapy.
