Suggest Treatment For Recurrent Escherichia Coli Infection

Hello Doc, A 78 yr female, with recurrent E.coli infection. she is on Nitrofurantoin macrocrystals 100 mg PO as a prophylaxis for almost a yr. How long she can be on Nitro or does she can switch to TMP/SMX low dose1 tab of 1/2 tab qhs 3 times weekly or what is the other option? Thx

Hi,
If she is comfortable on nitrofurantoin, there is no need to change it. Bactrim is about as good as nitrofurantoin, but f you are looking at alternatives, cephalexin 250mg at bedtime is a good one. Levofloxacin 250 mg is also an option.
