I have gradually accumulated, so to speak, severe pain in the muscle next to my left scapula. I have injured muscles my whole life due to athletics, work, and more work. This time I have something very different. When I get still and am not using my back muscles, generally when I lay down to sleep, they get extremely painful and I get tingling down the back of my left arm and the outsides of my two left fingers on my left hand. This tingling has me concerned. This pain is extreme and muscle relaxers do not help at all. What am I dealing with here ?

As per your explanation here I believe you have develop issues with your cervical spine as well the disc. I suggest to do mri of cervical spine and get it checked by orthopedic, he will suggest more and explain you what has happen to your spine. After taking medication advise. He may give surgical advise for severe cases. do visit physio once. they have some instrumental treatment like ultrasound therapy as well also for pain management heat therapy can be given. They will also guide you on posture as well for strengthening exercises. you can follow all this along with hot and cold pack use at your painful area frequently in day time at home. I am sure you will be fine with this. Incase if pain is too much and not be tolerable orthopedic doctor will suggest you further on injection in spine for temporary relief and surgical option for long term relief. Take care.
