Is this serious: L4-L5: Disc bulging and moderate facet arthropathy with ligamentum flavum hypertrophy causes severe central canal stenosis compressing the thecal sac to a diameter of 6-7 mm. Severe bilateral neural foraminal stenosis. L5-S1: Mild facet arthropathy. Severe bilateral neural foraminal stenosis.
There are a few options to treat this, which will be based on your exam and pain pattern. I would always start with physical therapy and steroid injections to see if that makes your pain levels manageable. If not, surgery might be recommended. With stenosis and mild to moderate facet disease, I'd probably start with a minimally invasive decompression. If you have instability on imaging, a fusion might be necessary.
