Suggest Treatment For Stress And Stomach Bloating

Dear Doctor,

I am 28 year old male. For the past 4 years my Job role has become mostly sedentary but highly stressful. I started putting on weight slowly, say 1.5-2 kgs every year. Now I am 73kgs and 173cms in height. For the past 3-4 years I have a peculiar problem, i.e. immediately after having some food I have the urge to pass stools, surprising thing I observed is whether I eat heavy/light food in night next day morning I pass the same quantity of stool and I feel as if I had emptied by stomach, I also observed that what I eat in the meal is seen immediately in the stool. but if I drink even a cup of coffee within few minutes I have the urge to pass stools and I end up passing them in same quantity. I infact tried having only one meal a day and only coffee for the rest of the day, but I ended passing stool more than 3-4 times with huge quantity only...... Simultaneously my tummy started bloating gradually, its always pot shaped and feels always full, whether I eat or not eat. when I don't have any pain in stomach, stools are normal. no diarrhoea... Also I observed that in last three years the frequency of me passing gas has increased. (Apprx 5-6 times a day, non foul smelling) I am vegetarian and I rarely consume Egg, also I have severe loss of appetite as well and I usually skip by breakfast. Apart from this I catch cold for every other day with running nose/nasal block.... I smoke on an average 4-5 cigrattes (Not a pack) a week, very rare social drinker..
Please suggest me what could be the problem and what should I do for this ?

I have read your full query and understand the concern.

This symptoms seem to be due to irritable bowel syndrome from your whole description.

Stomach bloating could also be related to to irritable bowel or may be gastroenteritis but needs to be evaluated further by various tests.

Now,if it is irritable Bowel which could be a very long term issue and needs constant therapy.

However, there is no permanent cure for the problem and it's management relies on diet and lifestyle modifications along with certain medications.

Diet and Lifestyle Modifications in IBS like avoiding Pulses and Beans, food with high amounts of refined sugars will help.

Good stress management and yoga have all been used to help manage the symptoms of IBS with good results.

Medications in IBS like loperamide (Brand Name: Eldoper) has been used in the management of IBS with diarrhea (many bowel movements a day, like yours).

Antispasmodics like mebeverine (Brand Name: COLOSPA) has also been used to regulate the bowel in IBS.

Lastly, Tricyclic Antidepressants (TCA's) like Amitryptalline have been shown in studies to help with IBS.

Therefore,you need to see physician or preferably gastro-physician who will examine you with some relevant tests and come to a certain conclusion.
