My advice in here is that you proceed with the following steps:
1) Do these lab tests: CBC,
ESR, CRP to rule out any possibility of infection in the metal that you have.
2) Do an X-ray for your knee (to make sure that the metal inside is well properly aligned.)
3) Hip X-ray is needed (some patients can present with a
knee pain even though they have a pathology primarily located at the hip.)

After we make sure that you don't have abnormalities in the things that I have mentioned above we proceed with the examination of the back to rule out any pathologies that can be primarily there
I want you to know that we need a stepwise approach to your problem so we can help you, especially that you mentioned you have a complicated course with your total knee (
stiffness post surgery) which in some instances means that there is a lack in
physiotherapy post operation.
So proceed with the steps that I have mentioned above and in the meantime to lessen your pain while investigating the problem try to take some over the counter simple analgesics if you don't have contraindications for them and apply some ice packs over the knee for 10 minutes 2-3 times a day and avoid ascending and descending the stairs.
