The wound seems to have developed secondary bacterial infection with involvement of the surrounding skin and soft tissue; leading to swelling, pain and
fever.
In such cases it is imperative to start with empirical broad spectrum antibiotics and control the spread of infection as soon as possible. The route of administration of antibiotics is governed by the extent and severity of infection.
IV route is preferable for faster control and rapidly spreading infection and in those with systemic symptoms such as fever.
Nevertheless, Oral
cefixime is a good choice as it covers for both gram +ve as well as gram -ve bacteria.
Apart from
oral antibiotics the wound should be cleaned for any pent up pus and dead tissue debris along with daily antiseptic dressing.
Betadine solution can be used for cleansing the wound. Betadine being antibacterial, antiseptic is helpful in preventing and treating bacterial contamination.
You may take the help of a nurse for wound cleansing and antiseptic dressing.
Also monitor for temperature, fluid intake.
If the condition does'nt seem to improve in another 2-3 days and fever continues, it would be better to get him admitted and administer antibiotics by IV route.
