My 7yr old Brother had a tooth abses in his upper moler, and they removed the tooth 4 hours after being on antibiotics. 5 days passed and his face is swollen and he s having a little bloody nose very sleepy and in pain. Could the infection have gone into the other moler?

Sounds like your brother has developed, acute maxillary sinusitis. It can rapidly spread and cause fever. Please take him to ER for proper treatment.
The upper molars are seperated from maxillary sinuses by a very thin membrane, which can get damaged by tooth extraction. As I said , this condition can be only diagnosed after examination.
