Orthopaedic Surgeon 's  Response
Hello,
Welcome, and thanks for sharing your concern
I went through your query, and I feel, the first advise is to stop on smoking and second to stop on alcohol, in this sequence. smoking can cause peripheral vascular disease, where the small arteries stop sending blood and this can lead to problems in circulation and gangrene.
Second see a doctor to have a look at your complains, and see if things can get better.
I hope my advice would have been useful, in decision making regarding your treatment, still if you have any clarifications or doubts feel free to contact back.
Thanks.
