If you had unprotected sex - need to rule out sexually transmitted infection. If not, it can be due to prostate inflammation or orchitis - share your lab reports. Masturbation 2 to 3 times a week is considered normal, more than that can cause nutrition issue can give you pain in penis during masturbation / sex.
Night fall / nocturnal emission can be controlled by decreasing the sexual thoughts - yoga, exercise and change of diet pattern.
