Hi. Sorry to hear about your condition and it seems like you are suffering from a bad case of acute bacterial pharyngitis or pharyngotonsillitis.

The severe throat pain and difficulty in swallowing is due to the pharyngitis. The pain in the neck from the pharyngitis can radiate to the ears due to similar nerve pathways that carry pain. The infection in the throat can ascend up to the back of the nose and block the eustachian tube that opens into the middle ear. This can explain the associated ear blockage and feeling of fluid being collected within the ears. Neck stiffness is probably due to enlarged and tender cervical lymph nodes.
The chills and sweats with blood tinged phlegm are indicative of bacterial pathology (? streptococcus).

You need a good course of antibiotics , anti inflammatory medications, antiseptic gargles, and decongestant medications. These can be prescribed only after you have been evaluated by an ENT specialist. Considering your state with poor recovery since 3 days, it may even be more appropriate for you to be admitted for IV medications, IV fluids and blood work up (cell counts and cultures). It is my strong recommendation that you visit a hospital at the earliest and have your self evaluated at the ER.

Hoping for a speedy recovery.
