Suggest Treatments For Low Fever, ear Infection With Pink Dicharge

My 7 year old son who is prone to ear infections is currently complaining of ear pain and also has a mild elevated temperature. Last night, he was evaluated by an urgent care doctor but was told to wait on antibiotics. He was in extreme pain during the night and has continued to have a mild temperature during the day today. He has also had slightly pink discharge from the ear. When do you know it's time to treat with antibiotics?
