This appears to be a peripheral arterial disease in which there are blockages in the leg arteries and decreased blood supply to limbs leading to symptoms.
He should first get arterial Doppler of the lower limb vessel which will confirm the diagnosis and later we can start medicine.
Hope this helps you and get back if you have any doubts.
