What Are The Causes Of Severe Bleeding And Large Clots?

hi i have been excessively bleeding for ten days like while im laying i start pouring everytime i stand up i pour blood i have large clots when i potty i have to get up a lot during the night im weak fainty pale tired and back pain stomach pain . the week before i had pressure and light spotting and had the urge to urinate but only urinated a little but veryery often. what could cause this and do i need immediate attention and wat could happen if i dont get it.
