hi, 80y old female 5 6 130 lbs, on Coumadin with oscillating INR with known atherosclerosis and some partial vascular occlusions in both lower extremities now presenting with thickened scaly red skin some welling bilat. ankles and top of feet. can this be a beginning of skin necrosis from poor perfusion or dermaititis. pt on blood pressure meds and poorly hydrated, smoker

Regarding the Coumadine side effect the main one is bleeding, since it is a blood thinner its effect makes you more vulnerable to bleeding from minor trauma or other causes such as ulcer. As for the swelling and changes in your feet, I don't think they are related to Coumadine. It's had to pinpoint what they are without being able to physically examine you. They might be in the setting of venous insufficiency. Might be related to conditions of the kidney, the heart or the liver, for which tests are needed. Also you don't mention your blood pressure meds, some of them like Amlodipine can cause swelling of the feet.
