I suggest a possibility of
acne vulgaris.
Most of the patients who have acne do not have any detectable underlying hormonal abnormality.
Nevertheless, hormones (even normal levels) are one of the factors implicated in the pathogenesis of acne.
Other factors implicated in pathogenesis of acne are increased end organ sensitivity to hormones, sticky epidermal cells leading to cohesion of epidermal cells which may block the pores, a bacterium known as
Propionibacterium acnes and inflammation i.e cytokines and inflammatory mediators.
Itching is subjective and can be a symptom in some cases of acne, specially inflammatory acne.
However, most of the times acne is asymptomatic.
Considering the fact that she is pregnant, I would only suggest topical antiacne preparations for her.
She may use clindamycin gel during day time and
benzoyl peroxide gel during night.
Not all oral medicines are unsafe in pregnancy.
Ampicillin is safe in pregnancy and so is azithromycin and can be started for her inflammatory acne i.e
pustular acne, if her
dermatologist feels so.
I suggest a gentle cleaner for face wash e.g cetaphil cleansing lotion, twice daily.
I suggest her to visit a dermatologist for the needful.
