Hi, welcome to our site. I am Dr Saumya Mittal.

Read your query. That is a very significant question and i appreciate your problem. I will try my best to answer your query

The possibility seems to be high that he has developed what is termed as diabetic neuropathy and this has affected his autonomic nervous system. The idea of autonomic nervous systemn is to maintain the functions of body automatically. Like maintaining BO when getting up.

What happens is that when you get up from a chair, the blood pressure falls as the blood is drained to the legs by the effect of gravity. this fall in blood pressure causes vertigo and a possible fall.

I would suggest the following steps
1. Wiggle the toe before getting up.
2. Hold on to something before getting up
3. Plenty of fluids should be taken.
4. crepe bandages on lower legs may help. compression bandages may be used as well
5. maintain a good sugar control. Make sure he is not developing hypoglycemias

I hope this helps you. Inform the reports mentioned above so i can be of help further. Best of luck.
