What Causes A Hole In The Middles Of The Toe Nail?

I have a naturally darkened toe nail. One part of the toenail always looked aweful, so i clipped at it until i discovered a rusted brown area underneath. Now I have cut at it so much that i have a small hole in the middle of the nail. I have been to a foot doctor who said it wasnt fungus. Can someone suggest what I should do.
