What Causes A Wound In My Penis?

hi Doc. i have problem in my pennis doc please help me to solve it, im 24 years old,i have wound in my pennis and im suffering this for very long time, i keep my pennis clean and i wash it everyday with my male wash solution. i wander and im afraid of my situation because most of the time my pennis is very itchy,sometimes if want to masturbate or to have sex i cant because of this.please help me doc. i need your advice of what i am going to do.im looking forward to hear your advice very soon doc.thank you
