Differential diagnosis of left upper quadrant pain involves:
- Gastric ulcer and gastritis, renal colic or renal stones,
pyelonephritis,
diverticular disease,
inflammatory bowel disease,
ischaemic colitis, pancreatitis. These are some of the most common cause of pain in left upper quadrant. Few basic investigations should be done before initiating the treatment.
- Blood tests renal and liver function, glucose, amylase, calcium, hepatitis serology, ESR/CRP. Urine microscopy and culture; pregnancy test if appropriate. X-rays: CXR (erect chest if there is suspected perforation.
- look for air under the diaphragm). Plain
abdominal X-ray. Kidney-ureter-bladder (KUB) film for renal colic (although CT KUB may be preferred).
However as your pain is getting worse on eating and is related to food intake it could be a peptic ulcer or acid peptic problem. Take tab Pantoprazole if not relieved then upper G.I endoscopy has to be done.
