What Causes An Elevation In The Blood Sugar Level Despite Taking Metformin?

My blood sugar has been consistently running about 300-369 for a couple of months with an A1c of 12.7 My doctor put me on 500 mg metformin a day and when I went back it was still 347. He increased the dose to 1000mg twice a day. the lowest I've gotten it it 276 fasting this am. I have been really forgetful and irritable. What else should I do, I have given up sweetened beverages and cut carbs a lot.
