What Causes Bleeding Bowel Movements?

My grandson is 3 years old, he can't push when moving his bowel and developed blisters that bleeds when you wipe gently and crying everytime you clean him up. Sine he does not know or scared to move his bowel due to pain/sting, tried to move his bowel more than twice a day. Balmex is the ointment that we are applying and it's not clearing up. It seems it's getting worst.

- Laxative as lactulose monohydrate , digestive enzymes prescription is
mandatory with his pediatrician .
- Check that he is chewing his food thoroughly .
