Noted your history of blood in poop, poop comes out in small diameter, pain on the left side in kidney area which increases on eating, constant cough for over 1 year, pains like a pinch as you have explained. Splotches on chest area. Your history is suggestive of either a fissure and/or hemorrhoids due to difficulty in passing poop. The pain on food can be due to reflex pain of colitis or due to gastritis. I would advise you the following: First of all, get clinical evaluation, examination, per-rectal digital examination, ano-ptoctoscopy and colonoscopy with biopsy to get a probable diagnosis to get started on a proper treatment. Upper GI endoscopy to rule out gastritis or such issues as you have pain on food. Ultrasonography of the abdomen and CT scan of the abdomen if indicated so. The tests of stool, blood and urine adds to the clinical picture to get a proper diagnosis which lead to proper treatment. Till then have Sitz bath, take laxatives and symptomatic anti-spasmodic treatment.
